What breaks a fast? Coffee, tea and more by fasting goal

Black coffee, a splash of milk, sweeteners, BCAAs: not everything breaks a fast in the same way. It depends on whether you want to protect insulin, autophagy or your weight.

The question is rarely just: does this break my fast? The better question is: which effect of my fast do I want to protect right now? Blood sugar follows different rules than cellular self-cleaning. And your weight follows different rules again.

It's just after seven. You haven't eaten since dinner. Fourteen hours, almost there. The coffee machine is running, and you're standing at the fridge. The milk already in your hand.

And then this thought: am I ruining everything right now?

If you google that question, you end up between two camps. One says: anything other than water breaks your fast. The other says: under 50 calories, nothing matters. Both sound very sure of themselves. Both leave out something important.

Here's how I see it: fasting is not a single switch. It's a bundle of processes. They start at different times and respond to different signals. A splash of milk may hardly matter for one process and matter quite a bit for another. That's why you won't get a rigid yes/no list here. You'll get a map.

What breaks a fast? Start by asking about your goal

Maybe you fast because you want to lose weight. Maybe because of your blood sugar. Maybe because you've read about autophagy, the self-cleaning of your cells. Or simply because the break does you good.

That's not a side issue. Each of these goals depends on a different signal in your body. And a drink can barely touch one signal while noticeably shifting another.

Goal 1

Insulin and blood sugar

Insulin is your body's warehouse manager. As long as it's on duty, things get stored rather than released. Sugar and certain protein building blocks can call it in.

Goal 2

Autophagy

Imagine a recycling centre that opens mainly when no new deliveries are arriving. Amino acids are considered the strongest signal for closing it again.

Weight and energy

Here, what counts most is your energy balance over the day. Like a bank statement: every transaction goes in, even the small ones.

Your hormonal system sends the commands via insulin and adrenaline. Your nervous system responds to caffeine and to stress. And your immune system is closely linked to your gut, which can respond to sweeteners. No drink touches just one of them.

Fasting isn't a light switch that's either on or off. It's more like a dimmer with several sliders. The question “Does this break my fast?” becomes more honest when you ask: which slider am I turning down right now?

And now you know why two people can give you two different answers about the same cup of coffee. They're thinking of two different goals.

Does black coffee break a fast?

For many people, the morning coffee is the anchor that makes fasting possible in the first place. Without it, the morning would feel pretty long. All the more unsettling is the thought that this very coffee might undo everything.

The short version: black coffee provides only a few kilocalories, practically no sugar and no meaningful protein. Based on current knowledge, it doesn't clearly break any of the three goals. It gets interesting when you ask what else coffee and caffeine might set in motion.

Animal studyCoffee and autophagy

A research team led by Pietrocola and Kroemer in Paris gave mice coffee, with and without caffeine. Within one to four hours, autophagic flux rose in the liver, muscle and heart, accompanied by inhibition of the growth switch mTORC1. What this means for you: coffee might even support rather than disturb your fasting window, but so far this has only been shown in mice.

What's interesting: decaffeinated coffee showed this effect too. The researchers therefore suspect polyphenols, plant compounds in coffee, as a possible trigger. This hasn't been studied in humans. I consider it biologically plausible, but no more than that.

Clinical studyCaffeine and insulin sensitivity

Keijzers and colleagues in Nijmegen gave twelve healthy people caffeine intravenously and measured insulin sensitivity using the elaborate clamp method. Insulin sensitivity fell by 15 percent, and adrenaline in the blood rose fivefold. What this means for you: caffeine can influence your insulin response in the short term via your stress system, even though a cup of coffee is not the same as an infusion.

This is where the nervous system comes in. Caffeine blocks the docking sites for adenosine, your brain's tiredness signal. The sympathetic nervous system kicks in, and adrenaline rises. That can mobilise fat from your stores. But in sensitive people it can also trigger palpitations, inner restlessness or stomach discomfort, especially on an empty stomach.

Clinical studyCaffeine and ketones

A team led by Vandenberghe and Cunnane in Sherbrooke, Canada, gave ten healthy adults two different doses of caffeine with breakfast. Ketone production rose in a dose-dependent way by 88 and 116 percent, and free fatty acids in the blood increased as well. What this means for you: caffeine appears to give fat metabolism a push, although the measurements were taken with breakfast and not in a fasted state.

And tea?

Unsweetened green, black or herbal tea contains practically no calories, no sugar and no protein. There are hardly any direct studies on tea during the fasting window. My assessment is therefore based on the logic of the signals: where there's no sugar and no protein, there's little to call in insulin or mTOR. Green and black tea contain caffeine, so the same applies to them as to coffee.

Things look different as soon as honey, syrup, sugar or sweetened ready-made teas end up in the glass. Then it's no longer a fasting drink but a small snack.

Change of perspective

Based on current knowledge, black coffee is not a fast breaker. But it isn't a fasting booster either, just because a mouse study suggests it. If it makes you jittery or your stomach rebels, that's a signal from your nervous system that you're allowed to take seriously.

And now you know why black coffee keeps its firm place in most fasting approaches.

Does a splash of milk break a fast?

This is where it gets interesting. Because milk is more than a bit of colour in your coffee. It contains lactose, fat and milk protein, meaning whey and casein. And protein in particular is a signal your body registers very precisely.

The idea behind this section: milk can trigger more insulin than its sugar content would suggest. The reason appears to lie in the milk protein.

Human studyMilk and insulin

Hoyt, Hickey and Cordain in Fort Collins, USA, measured the blood sugar and insulin response to whole and skimmed milk in nine healthy adults. The glycaemic index was 42 and 37, while the insulin index was 148 and 140. What this means for you: milk can stimulate insulin considerably more than its effect on blood sugar would suggest, and this is independent of fat content.

Clinical studyWhey protein as an insulin signal

Nilsson, Holst and Björck in Lund, Sweden, gave twelve healthy people glucose drinks with whey protein or with free amino acids. With whey, the insulin area was 60 percent larger than with pure glucose, and a mixture of branched-chain amino acids also clearly increased the insulin response. What this means for you: milk protein appears to be an insulin signal in its own right, not just the sugar.

Now the honest caveat: both studies tested whole portions, not a splash in your coffee. As far as I know, a splash of two or three tablespoons has never been specifically studied. The effect is probably small then. Whether it's relevant for you once again depends on your goal.

  • Insulin: a small stimulus. Probably minor with one splash, more likely noticeable with three milky coffees over the morning.
  • Autophagy: milk protein supplies amino acids. Exactly those are considered opponents of cellular cleaning.
  • Weight: a splash of milk barely matters. It counts, but it doesn't decide your success.

And cream? Per spoonful it contains more fat and less lactose and protein than milk. It's plausible that it stimulates insulin less, but this hasn't been specifically studied. With plant-based drinks, it's worth checking the label: oat drinks often come with carbohydrates, unsweetened almond drinks hardly any.

Change of perspective

A splash of milk doesn't break your fast the way breakfast does. But it isn't nothing either. If your goal is insulin or autophagy, you can save the milk for your first meal. If your goal is weight and the splash of milk makes fasting easier for you, it can be a fair compromise.

Sweeteners while fasting: stevia, sucralose and aspartame

The diet cola in the afternoon. The sweetener in your tea. Zero calories. So it doesn't matter? It's not quite that simple. But it's not as dramatic as some posts make it sound either.

The idea: sweeteners on their own appear to barely move blood sugar in the acute phase. But in combination with sugar, and via the gut, they might shift something in some people.

Meta-analysisSweetened beverages on their own

Zhang, Sievenpiper and colleagues in Toronto analysed 36 studies with 472 mostly healthy people. Sweetened beverages drunk on their own changed blood sugar, insulin and gut hormones acutely about as little as water, though with only low to moderate certainty of evidence. What this means for you: based on current knowledge, a sweetened drink in your fasting window hardly calls in insulin.

Clinical studySucralose before sugar

Pepino and colleagues in St. Louis gave 17 people with severe obesity, who otherwise did not use sweeteners, sucralose or water ten minutes before a sugar solution. With sucralose, the insulin area was 20 percent larger and insulin sensitivity was 23 percent lower. What this means for you: sweetener shortly before breaking your fast might change the response to your first meal.

A second study from Mexico City with 137 young, healthy adults fits this picture. After ten weeks of 48 milligrams of sucralose a day, insulin levels in the sugar test were higher than at the start. The authors themselves emphasise that the effect did not follow the dose neatly and that more research is needed.

Clinical studySweeteners and the microbiome

Suez, Elinav, Segal and colleagues at the Weizmann Institute in Rehovot gave 120 healthy adults saccharin, sucralose, aspartame or stevia for two weeks, each below the acceptable daily intake. All four changed the microbiome, and saccharin and sucralose also impaired glucose tolerance, in ways that varied greatly from person to person. What this means for you: how your metabolism responds to sweeteners might depend heavily on your gut.

The same research group had already shown in mice in 2014 that sweeteners could trigger glucose intolerance via altered gut bacteria. When the researchers transferred the gut contents to germ-free mice, the disturbance was transferred too. These are animal data, but they explain why the immune system and the gut have a say in this topic.

And stevia? In a study from Baton Rouge with 31 people, preloads with stevia lowered the subsequent rise in insulin more than those with aspartame or sugar. So not all sweeteners are the same.

Change of perspective

When fasting, sweetener is less a question of calories than a question of combination, habit and gut. A glass now and then is something different from a litre every day. And if you notice that the sweet taste wakes up your appetite, that observation counts for more than any table.

BCAAs, protein and bone broth: the mTOR switch

People who train fasted in particular like to reach for BCAAs, the branched-chain amino acids. No sugar, hardly any calories, perfect for fasting. Right?

The idea: for the autophagy goal, amino acids are among the strongest counter-signals, even in small amounts. Few calories doesn't mean little effect on your cells here.

Imagine mTOR as the site manager in your cells. When building materials are available, it calls out: build! When they're missing, it's time to tidy up and recycle. The tidying up is autophagy. And the most important building materials the site manager responds to are amino acids.

Mechanism reviewLeucine and mTORC1

Dodd and Tee from Cardiff summarised how cells sense the supply of amino acids and report it to mTORC1. According to their review, leucine alone may be enough to switch on the mTORC1 signalling chain, which controls autophagy as well as protein building. What this means for you: a BCAA drink in your fasting window might flip exactly the switch you actually want to leave at rest for cellular cleaning.

Then there's insulin. In the Lund study, a BCAA mixture taken together with glucose clearly increased the insulin response. How strongly BCAAs call in insulin without any sugar is less clear. For weight, a few grams of amino acids hardly play a role. So the picture is mixed, and that's exactly what this article is about.

Bone broth

Bone broth provides protein in the form of collagen and amino acids, plus salt and a little energy. Direct studies on broth during the fasting window are lacking. According to mTOR logic, it's rather unfavourable for the autophagy goal. Some fasting programmes still use it deliberately, as a warming compromise with salt. In that case it's a conscious decision, not an accident.

Change of perspective

Few calories doesn't mean little signal. BCAAs are almost invisible on the bank statement, but for the site manager mTOR they're a loud message. If cellular cleaning matters to you, they belong in your eating window, not before it.

Coconut oil, MCT oil and butter coffee

Butter coffee, often with MCT oil, promises long-lasting fullness and a clear head. Many people use it to go longer without breakfast. Does it break a fast?

The idea: pure fat appears to stimulate insulin release considerably less than sugar or protein. It still provides plenty of calories, though. One tablespoon of oil contains roughly 120 kilocalories, and a butter coffee can quickly add up to considerably more.

Human studyMCT oil and ketones

Vandenberghe, Cunnane and colleagues in Sherbrooke compared different oils in nine healthy adults, each emulsified in milk and in some cases taken with breakfast. Pure C8 MCT oil raised ketones the most, while coconut oil reached only about a quarter of that peak value. What this means for you: coconut oil and MCT oil are not the same when it comes to ketone production.

For the three goals, this means: insulin probably stays fairly calm. Whether fat in coffee slows autophagy in humans has not been studied. For weight, the calories count fully. In terms of energy, a butter coffee can be a small breakfast.

Change of perspective

Butter coffee isn't fasting in the strict sense. But it can be a bridge if you want to shrink your eating window step by step. Bridges are good, as long as you know you're standing on one.

Autophagy: why nobody can give you an exact threshold

Maybe you've read numbers. Autophagy starts after 16 hours. After 24 it's in full swing. A splash of milk stops it immediately. That sounds precise. Honestly, in humans it can hardly be verified.

The idea: so far, autophagy has been difficult to measure in humans. Any exact statement about what “breaks” it in humans is therefore an inference from cell and animal studies.

Study protocolMeasuring autophagy in the blood

Bensalem, Sargeant and colleagues in Adelaide write that human data on autophagy are limited because autophagic flux has been hard to measure in humans. They therefore developed a blood test and planned to use it to study the effect of a whey protein drink after an overnight fast. What this means for you: research is only just beginning to ask the question “What breaks autophagy?” directly in humans.

A 2025 review by Moreno, Nieto-Torres and Kumsta comes to the same conclusion. Autophagy is a multi-step process and particularly difficult to capture in humans. New cell models based on blood cells and skin cells are meant to change that.

Well established

Sugar and milk protein can clearly stimulate insulin. Calories count for your energy balance. Blood sugar lowering medication combined with fasting can increase the risk of hypoglycaemia.

Mechanistically plausible

Amino acids such as leucine can activate mTORC1 and thereby slow autophagy. Coffee might stimulate autophagy, as shown in mice.

Still open

At what amount of milk, broth or fat autophagy measurably drops in humans. Exact numbers of hours for when autophagy begins.

Change of perspective

If someone gives you an exact autophagy threshold in calories, you're allowed to ask kindly how it was measured. That isn't criticism of research. It's respect for what we don't know yet.

And now you know why I phrase things more cautiously for autophagy than for insulin. For insulin, we have measurements. For autophagy, so far we mainly have good reasons.

About the 50-calorie rule The often-cited limit of 50 calories is a rule of thumb from the fasting scene. I'm not aware of any study that establishes exactly this number as a threshold. What matters more is the type of signal: sugar and protein send different messages than pure fat.

Why the time window counts more than the spoonful

Maybe you know this feeling: you've been fasting for weeks, everything is going well. Then one day a splash of milk slips into your coffee, and suddenly the whole day feels lost. That kind of strictness takes a lot of energy.

The idea: for many metabolic effects, the rhythm of eating appears to matter more than a single spoonful in the fasting window.

Clinical studyEarly eating window

Sutton, Peterson and colleagues in Baton Rouge had men with prediabetes eat within a six-hour window with dinner before 3 p.m. for five weeks, alternating with a twelve-hour window for comparison. The early, short eating window improved insulin sensitivity, blood pressure and oxidative stress, even though the men ate enough to maintain their weight. What this means for you: timing itself might be a lever of its own, independent of weight loss.

Fasting isn't an exam you fail with a splash of milk. It's a break in which your body can learn to stay stable between meals. Energy in the morning without thinking about the next snack. That's the real freedom behind it.

Change of perspective

Perfection in the fasting window is not the goal. A rhythm you can keep up for months will probably do more for you than three perfect days after which you give up.

When fasting and fasting drinks call for caution

For all the calm, there are situations in which the question “Does this break my fast?” becomes secondary. Then it's about safety.

Clinical studyFasting with diabetes medication

Corley, Krebs and colleagues in Wellington, New Zealand, followed 41 adults with type 2 diabetes taking blood sugar lowering medication for twelve weeks, with two very low calorie days per week. Fasting increased the rate of hypoglycaemia despite reduced medication, with a relative risk of 2.05. What this means for you: if you take blood sugar lowering medication, you should only start fasting under medical supervision.

Please discuss with a doctor first
  • If you take insulin or other blood sugar lowering medication.
  • During pregnancy and breastfeeding.
  • If you have or have had an eating disorder. Fasting rules can trigger old patterns again.
  • If you are underweight, and for children and adolescents.
  • If you take medication that has to be taken on an empty stomach or with a meal.
  • If coffee on an empty stomach causes you discomfort.

Never postpone or pause prescribed medication on your own just to avoid breaking your fast. Talk to your treating doctor beforehand. Fasting can wait. Your safety comes first.

Three levers for your next fasting window

You don't have to change everything tomorrow. These three steps are enough to turn uncertainty into a conscious decision.

Your three levers

  1. Set your goal before the coffee machine starts. Is today about insulin, autophagy or your weight? The answer decides whether the milk goes into your coffee.
  2. Think in three zones. Clear: water, black coffee, unsweetened tea. Grey zone: a splash of milk, sweetener, a spoonful of cream, chosen consciously rather than in passing. Rather red for the autophagy goal: BCAAs, protein shakes, broth.
  3. Observe your own response. How are you doing with hunger, restlessness and sleep? If you already wear a glucose sensor, you can see how your body responds to your milky coffee. Your data are more personal than any list.

And now you know why the question “What breaks a fast?” doesn't need a yes/no answer, but a question in return: which goal do you want to protect?

Frequently asked questions: what breaks a fast?

Does black coffee break a fast?

Based on current knowledge, black coffee does not clearly break a fast for any of the three main goals. It provides only a few kilocalories and practically no carbohydrates or protein. In a mouse study, coffee even appeared to stimulate autophagy, but this has not been shown in humans. Through adrenaline, caffeine can slightly lower insulin sensitivity in the short term and may cause restlessness or stomach discomfort in sensitive people. Without sugar, milk and syrup, black coffee is therefore considered one of the most fasting-friendly companions.

Does a splash of milk in coffee break a fast?

A splash of milk does not break a fast the way a meal does, but it is not neutral either. Milk contains lactose and milk protein, and in studies milk triggered considerably more insulin than its sugar content would suggest. Those studies tested larger amounts, and a single splash has hardly been studied so far. For the weight goal, a splash barely matters; for the autophagy goal, the milk protein supplies amino acids. If you want to protect insulin or autophagy, it may be better to leave the milk out during your fasting window or save it for your first meal.

Can I use sweeteners while fasting?

Sweeteners provide no calories, but they are not metabolically neutral in every situation. A network meta-analysis of 36 studies found that sweetened beverages drunk on their own affected blood sugar and insulin acutely about as little as water. Individual studies, however, showed that sucralose taken before a sugar load can increase the insulin response. In a study of 120 adults, saccharin and sucralose changed glucose tolerance in some people via the microbiome. For the weight goal, sweeteners are usually not a concern; for your gut and insulin, using them sparingly may be worthwhile.

Does tea break a fast?

Unsweetened tea is considered fasting-friendly because it contains practically no calories, carbohydrates or protein. This applies to green, black and herbal tea. There are hardly any direct studies on tea during the fasting window; the assessment is based on the logic of calories and insulin signals. As soon as honey, sugar, milk or sweetened ready-made teas come into play, the picture changes. Green and black tea contain caffeine, which can be unpleasant on an empty stomach if your stomach is sensitive.

Do BCAAs or amino acids break a fast?

For the autophagy goal, BCAAs are among the clearest fast breakers, even though they contain few calories. The amino acid leucine alone can activate the growth switch mTORC1, and mTORC1 is considered a central opponent of autophagy. In combination with sugar, branched-chain amino acids also triggered a stronger insulin response in one study. For the weight goal alone, BCAAs barely matter. If autophagy is important to you, BCAAs are better placed in your eating window.

Does coconut oil or MCT oil break a fast?

MCT oil and coconut oil probably stimulate insulin release only a little, but they provide a lot of calories. One tablespoon of oil contains roughly 120 kilocalories, which counts fully toward the weight goal. In one study, pure C8 MCT oil was considerably more ketogenic than coconut oil; coconut oil reached only about a quarter of the peak ketone level. Whether fat in coffee slows autophagy in humans has not been studied. Butter coffee is therefore more of a bridge to fasting than fasting in the strict sense.

Does bone broth break a fast?

In the strict sense, bone broth breaks a fast because it provides protein in the form of collagen and amino acids, as well as calories. For the autophagy goal it is therefore rather unfavourable, because amino acids can stimulate the mTOR switch. Direct studies on bone broth during the fasting window are lacking, however. Some fasting programmes deliberately use broth as a compromise, for example because of the salt and warmth. Whether that suits you depends on your goal and your condition.

How many calories does it take to break a fast?

There is no scientifically established calorie limit above which a fast is broken. The often-cited 50-calorie rule is a rule of thumb from the fasting scene, not a figure from studies. What matters is less the amount than the type of signal: sugar and protein send different signals to insulin and mTOR than pure fat does. For the weight goal, on the other hand, calories over the day really do count. So first ask yourself which fasting effect you want to protect.

Can I measure whether my autophagy is running?

In everyday life, autophagy in humans cannot yet be measured reliably. Researchers explicitly describe measuring autophagic flux in humans as difficult and are only now developing blood tests for it. Most statements about autophagy and fasting come from cell cultures and animal models. Time frames such as “from 16 hours” are therefore rough guidance, not measurements. Blood sugar, insulin and ketones, on the other hand, can be measured.

Can I take medication or supplements during my fasting window?

You should never postpone or skip prescription medication on your own just to avoid breaking your fast. Especially with blood sugar lowering medication, fasting can increase the risk of hypoglycaemia. In a study of people with type 2 diabetes, the rate of hypoglycaemia rose during fasting despite dose adjustments. So talk to your treating doctor beforehand. Fat-soluble supplements such as vitamin D are usually absorbed better with a meal in your eating window.

Transparency note: for insulin and blood sugar, there are clinical studies in humans. The statements on autophagy, by contrast, rely mainly on cell models, animal studies and mechanisms, because autophagy has so far been difficult to measure in humans. These connections are biologically plausible but not yet established with the same certainty as through large randomised trials. For tea, cream and bone broth in the fasting window, direct studies are lacking; here I describe assessments based on physiology. This article does not replace medical advice.
